What is it?
Dianella Health's Support Coordinator will help you access the supports and services you want and need to live a fulfilling and happy life under the NDIS.
How do I know if I need a Support Coordinator?
Once the NDIS has confirmed you meet the access requirements, a planner will arrange to meet with you. Together, you will discuss your needs, goals, aspirations & the supports you currently receive. This may take more than one meeting, you can bring someone with you for support. Using this information, your planner will then develop a plan of supports that best meets your needs & your goal.
What are the steps involved?
Once funds are available your plan is sent via DHHS to the Support Coordinator. Your Dianella Support Coordinator will set a meeting with you & provide you with information on what services are available & which organisations provide them, this is where the you choose which services & organisations you would like to engage with.
Your Support Coordinator will;
- ensure services meet their obligations,
- review your plan if you feel its not meeting your needs or if your situation changes,
- liaise with your support services,
- liaise with NDIA,
- review your plan annually,
- strengthen & enhance your abilities to coordinate supports & participate in the community. Top of Page ⇑
