Population Health and Health Promotion activities in Hume
The Population Health team delivers a range of health promotion initiatives and projects in Hume to create a healthy, safe and connected community. Collaboration with organisations and individuals form the foundation of our work, with our practice informed by the Victorian State Government's guiding principles for integrated health promotion, as well as the identified health needs of the City of Hume.
We are also heavily involved in the collaborative project Shared Vision For the North (SVN). SVN combines local policy development, planning and service provision agencies in outer northern Melbourne (Hume, Whittlesea and Mitchell) to develop integrated multi-agency approaches to service provision, to improve the health and well-being of the northern community.
